Health Kids Survey to all 7th grade students will be on Tuesday, November 15 and Wednesday November 16. The below form is to be completed if you do not want your 7th grade student to participate. They will receive a pass to the library. This form was also sent home with students the week of October 31.

Altimira Middle School understands that in a world that demands more and more of our time, parents can’t always spend time at our office filling out forms and paperwork. That’s why we’re now providing easy and convenient access to enrollment and permission forms online. It’s now easier than ever to keep your childrens’ records up-to-date and ensure their safety at school.
Check back soon to find some of the most commonly used forms, including those for enrollment and field trip permission. Simply fill out the forms and mail or fax them in. Of course, you’re still always welcome to drop them off at our office.

- Lunch Program enrollment—Sign your eligible child up for our reduced cost and free lunch program.
- Field trip permission slips—Authorize your child to attend a class field trip without waiting for them to bring home a form.
- School enrollment forms—Fill out paperwork online to enroll your new student at Altimira Middle School.
- Student Health forms—Submit your child’s medical history and emergency contact information to our health office.
